Family membership

Health insurance provides insurance benefits not just to insured persons, but to their dependent family members. These family members are referred to as “dependents”. Family members must meet certain conditions related to the extent of “family relationship” and “income” before they are authorized as dependents.

  • Adding a family member
    At companies that use WAVE, you are required to add family members via the WAVE system.
    (You do not need to add family members already registered as tax dependents.)

Deadline: Within five days after the reason for adding the family member arises
Applies to: Insured persons for whom the number of dependent family members increases due to marriage, childbirth, etc.

You must attach the following documents, if applicable:

When naming a non-Japanese citizen as a dependent
  • Copy of resident card or special permanent resident certificate
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent
When naming a child as a dependent when the child has a family name other than that of the insured person
  • Official copy of family register
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent
When insured person names a child as a dependent (If both spouses work)
  • Your spouse’s salary details for the most recent three months (copy) or salary payment certificate for the most recent three months (original)

  • Other necessary documents, depending on the dependent
When insured person names a child as a dependent (If you are unmarried)
  • Official copy of family register
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent

To certify a family member as a dependent who does not receive unemployment benefits under employment insurance or who is going to extend the benefit receipt period

Removing a family member

Required documents: *Japanese version only
Health insurance card (for the dependent to be removed)
Elderly benefits card (if issued)
Documentation of date of attaining eligibility—for example, a copy of the health insurance card issued by the current health insurance association (if the person becomes a member of another health insurance association through employment or for other reason)
Deadline: Within five days after the reason for adding the family member arises
Applies to: [Insured persons with dependents who meet the following descriptions]
  • No longer eligible as dependents due to reasons such as employment, living apart from the insured person, or death
  • No longer satisfy the conditions for certification as dependents due to increased income
  • No longer rely on the income of the insured person for their livelihood because the insured person no longer provides an allowance

Apply from WAVE if you want to be issued a Certificate of Loss of Health Insurance Eligibility. Contact HR One Corporation if you have any questions or wish to apply after leaving your employer.
