Moving and job transfer

Do the following each time you change your address:

Change of address

[Current employees (insured persons)]
Notify your employer (company). You do not need to notify the Health Insurance Association.

  • Change of address on certificate of residence/mailing address: Apply from WAVE.
  • Change of current address: Apply from Workday.

[Family members of current employees (dependents)]
Submit a Notification of Address Change in either of the following cases:

  • When a dependent begins living apart from the insured person
  • When a dependent who lived apart from the insured person begins living together with the insured person

[Voluntarily and Continuously Insured Persons and their dependents]
Submit a Notification of Address Change to the Health Insurance Association.
Submit the notification even for changes of address (changes of home address) that do not involve changes in the certificate of residence.

Correct the address on the back of your health insurance card yourself if your address changes.

Required documents: *Japanese version only
Deadline: As soon as possible
Applies to:

Family members of current employees whose address has changed
Voluntarily and Continuously Insured Persons and their dependents whose address has changed

Notes:

If you no longer live with a dependent due to a change of address, check whether the dependent certification criteria are still satisfied.

[Spouse, child, parent, grandparent, great-grandparent, grandchild, or sibling]
If the dependent lives apart from the insured for reasons other than an unaccompanied job posting or school attendance, the dependent must be provided with a monthly allowance by the insured to remain a dependent.
You will be required to submit documentation of the allowance at the time of annual dependent status confirmation (certification).

[Parent in law, stepchild, or other family member within three degrees of consanguinity]
The above family members must be removed from your list of dependents if living apart, since living together is a condition of dependent certification.
