After you leave your employer

You will lose your eligibility for membership in the Health Insurance Association after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.

Return your health insurance card when you leave your employer

Required documents:
  • Health insurance cards (for the insured person and all dependents)
  • Elderly benefits card (if issued)
Deadline: Within five days after the date of loss of eligibility
Applies to: Insured persons leaving employment and their dependents

  • When you support your dependents continuously, submit documents below.
Deadline: Must be received within 20 days after the date of loss of eligibility of the insured person (day after leaving employment).
Applies to: Insured persons who have been insured continuously for at least two months prior to leaving employment

Notes: If the documents are not submitted within 20 days after the date of loss of eligibility (day after leaving employment), you cannot be qualified. (Must be received within 20 days.)

If you would like to cancel your status as a Voluntarily and Continuously Insured Person

If you start new employment and become an insured person under another health insurance plan

Once your new employer has been decided, notify the AIG Health Insurance Association by telephone or email of the date to start new employment.
You will lose your status as a Voluntarily and Continuously Insured Person on the date on which you are enrolled in your new employer’s health insurance plan.

Required documents: Application Form for disqualification as Health Insurance Voluntarily and Continuously Insured Person (for reemployment)

Voluntarily and Continuously Insured Person’s health insurance card (original)
Copy of health insurance card issued by your new health insurance association or other insurer

Deadline: Submit the above three documents to the AIG Health Insurance Association after receiving your new health insurance card.
Notes:  

For withdrawal through application by the insured person

If you would like to cancel your status to join the National Health Insurance, to become a dependent of a family member, or for other such reasons, submit the Application Form for disqualification as Health Insurance Voluntarily and Continuously Insured Person to cancel your status as of the first day of the month after the month in which the application form was received.

Examples:
If we receive your application on April 30, 2022: The date of disqualification will be May 1, 2022.
If we receive your application on May 1, 2022: The date of disqualification will be June 1, 2022.

  • * If insurance premiums are unpaid prior to the date of disqualification by the application
  • * Prepaid insurance premiums for the unused period will be refunded.

Voluntarily and Continuously Insured Person’s health insurance card (return after disqualification)
Send to: AIG Health Insurance Association
Notes: In principle, an application for disqualification cannot be retracted. Be sure to submit the above documents by post.
A Certificate of Loss of Health Insurance Eligibility will be issued on or after the date of disqualification.
