Adding a family member

Health insurance provides insurance benefits not just to insured persons but also to their dependent family members. These family members are referred to as "dependents". Family members must meet certain conditions related to "residency in Japan," "the extent of the family relationship," and "income" before they are authorized as dependents.

Adding a family member

At companies that use WAVE, you are required to add family members via the WAVE system.
(You do not need to add family members already registered as tax dependents.)

You must attach the following documents, if applicable:

When naming a non-Japanese citizen as a dependent
  • Copy of resident card or special permanent resident certificate
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent
When naming a child as a dependent when the child has a family name other than that of the insured person
  • Official copy of family register
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent
When insured person names a child as a dependent (If both spouses work)
  • Your spouse’s salary details for the most recent three months (copy) or salary payment certificate for the most recent three months (original)
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent
When insured person names a child as a dependent (If you are unmarried)
  • Official copy of family register
  • Notification of Health Insurance Dependent (Change)
  • Other necessary documents, depending on the dependent
