Health insurance cards are no longer newly issued or reissued since December 2, 2024. Use your My Number Card as your Health Insurance Certificate.

Procedures

To have a Health Insurance Eligibility Certificate, etc. issued or reissued

Applies to:	Insured persons and dependents who wish to have a Health Insurance Eligibility Certificate issued (reissued) for the following reasons: Lost Individual Number Card In the process of renewing Individual Number Card Individual Number Card electronic certificate has expired Individual Number Card issued but not registered for use as a health insurance card No Individual Number Card issued Individual Number Card returned Support from a third party (e.g., caregiver) required to receive examinations and treatment with a My Number Card as a Health Insurance Certificate Lost or damaged Health Insurance Eligibility Certificate

Applies to:	Insured persons and dependents who wish to have the Notice of Eligibility Information reissued due to loss or damage

Notes:	As a general rule, if you lose your health insurance card or Health Insurance Eligibility Certificate, you will be asked to use a Myna health insurance card.。 If you do not have a Myna health insurance card, submit an Application Form for Issue/Reissue of Health Insurance Eligibility Certificate. * This notification is also required if you are unable to return your health insurance card or other document due to loss or other reasons when you retire (or remove a dependent).

Notes:

As a general rule, if you lose your Health Insurance Eligibility Certificate, use your My Number Card as your Health Insurance Certificate (“Myna health insurance card”).
If you do not have a Myna health insurance card, submit an Application Form for Issue/Reissue of Health Insurance Eligibility Certificate.

* This notification is also required if you are unable to return your health insurance card or other documents due to loss or other reasons when you leave your employer (or remove a dependent).

If you lose your Myna health insurance card (Individual Number Card)

If your Individual Number Card is lost or stolen, take the steps required to disable its functions. Call the Individual Number Call Center general toll-free number (0120-95-0178).

If you would like to be issued a Health Insurance Eligibility Certificate for use until your Individual Number Card is reissued, submit an Application Form for Issue/Reissue of Health Insurance Eligibility Certificate.

In addition, if your individual number changes due to card loss, you will need to notify your employer of the new individual number.
