Notes: | New issue and reissue of current health insurance cards will be discontinued on December 2, 2024. For this reason, the Association will reissue health insurance cards for applications received by the Health Insurance Association by December 1, 2024. For applications received on or after December 2, the Association will conduct only card loss processing. |
If you lose your Myna health insurance card (Individual Number Card)
If your Individual Number Card is lost or stolen, take the steps required to disable its functions. Call the Individual Number Call Center general toll-free number (0120-95-0178).
If you would like to be issued an Eligibility Verification Certificate for use until your Individual Number Card is reissued, submit an Application Form for Issue/Reissue of Eligibility Verification Certificate.
In addition, if your individual number changes due to card loss, you will need to notify your employer of the new individual number.
