Applies to:

Insured persons and dependents who wish to have an Eligibility Verification Certificate issued (reissued) for the following reasons:

  • Lost Individual Number Card
  • In the process of renewing Individual Number Card
  • Individual Number Card electronic certificate has expired
  • Individual Number Card issued but not registered for use as a health insurance card
  • No Individual Number Card issued
  • Individual Number Card returned
  • Support from a third party (e.g., caregiver) required to receive examinations and treatment with a Myna health insurance card
  • Lost or damaged Eligibility Verification Certificate

Applies to: Insured persons and dependents who wish to have the Notice of Eligibility Information reissued due to loss or damage

Notes:

As a general rule, if you lose your health insurance card or Eligibility Verification Certificate, you will be asked to use a Myna health insurance card.。
If you do not have a Myna health insurance card, submit an Application Form for Issue/Reissue of Eligibility Verification Certificate.

  • * This notification is also required if you are unable to return your health insurance card or other document due to loss or other reasons when you retire (or remove a dependent).

If you lose your health insurance card, etc.

If you lose your Myna health insurance card (Individual Number Card)

If your Individual Number Card is lost or stolen, take the steps required to disable its functions. Call the Individual Number Call Center general toll-free number (0120-95-0178).

If you would like to be issued an Eligibility Verification Certificate for use until your Individual Number Card is reissued, submit an Application Form for Issue/Reissue of Eligibility Verification Certificate.

In addition, if your individual number changes due to card loss, you will need to notify your employer of the new individual number.
