As a general rule, if you lose your Health Insurance Eligibility Certificate, use your My Number Card as your Health Insurance Certificate (“Myna health insurance card”).

If you lose your Myna health insurance card (Individual Number Card)
If your Individual Number Card is lost or stolen, take the steps required to disable its functions. Call the Individual Number Call Center general toll-free number (0120-95-0178).
If you would like to be issued a Health Insurance Eligibility Certificate for use until your Individual Number Card is reissued, submit an Application Form for Issue/Reissue of Health Insurance Eligibility Certificate.
In addition, if your individual number changes due to card loss, you will need to notify your employer of the new individual number.
