Death

The Health Insurance Association will pay “Funeral Expenses” in the event of the death of an insured person or dependent. It will pay “Funeral Costs” to the person who actually administered the burial of a deceased person in the event of a deceased person with no family members or close friends.

If the insured person has died

Required documents:

*Japanese version only

Documents certifying the death
(copies of Death Certificate and Burial Permit or Cremation Permit)

*Not required to attach, when certification of employer is filled in the "Claim for Funeral Expenses".

Deadline:

As soon as possible

Applies to:

Dependent or person other than a dependent who relied on the insured person for a living (funeral expenses)
If no one relied on the insured person for a living, the person who actually undertook funeral procedures (funeral costs)

Notes:

When requesting payment of Funeral Costs, attach receipts indicating the cost of the burial, in addition to the Death Certificate.

* The receipt must be addressed in the name of the applicant for funeral costs.
* Be sure to attach the original receipt.
