You will lose your eligibility for membership in the Health Insurance Association after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.

Notes: | If you lose a health insurance card or Eligibility Verification Certificate that needs to be returned when you retire, submit a Notification of Health Insurance Card, etc. Loss. |
Caution
The health insurance card (or Eligibility Verification Certificate) will become invalid on the date of loss of eligibility. If a former Association member uses an invalid card or certificate by mistake, we may demand a refund of any medical care costs paid by the Health Insurance Association at a later date.
